- What AI systems are we operating? Do you have a complete inventory of AI tools in production β including ones adopted by individual departments without central approval?
- Who owns each AI system's risk? Is there a named owner responsible for monitoring performance, managing incidents, and ensuring appropriate use of every material AI system?
- What data are our AI systems trained on and processing? Is customer data involved? Is personal data used in model training? Have the data handling practices been reviewed by legal and privacy counsel?
- How do we detect when an AI system is producing harmful outputs? Is there a monitoring program? Who reviews model performance? What's the escalation path when something goes wrong?
- What would we do if we needed to shut down or roll back an AI system? Is there a documented incident response plan for AI failures? Have you tested it?
What an AI Governance Framework Actually Contains
A practical AI governance framework for a mid-market organization doesn't need to be a 200-page policy document. It needs to answer four things concisely:
2. Roles & Accountability β Who is responsible for approving new AI deployments. Who monitors performance. Who can authorize exceptions to policy.
3. Standards & Controls β What must be true about any AI system before it goes into production. Minimum requirements for testing, documentation, human oversight, and data handling.
4. Monitoring & Review β How often AI systems are reviewed for drift, bias, or performance degradation. What triggers an out-of-cycle review. How incidents are documented and reported.
The Difference Between Policy and Governance
A surprising number of organizations confuse publishing an AI policy with having AI governance. An AI acceptable use policy tells employees what they can and cannot do. Governance is the structure that ensures the organization's AI operations are actually aligned with its risk appetite β and that someone is accountable when they're not.
You can have a ten-page AI policy and zero governance. Governance requires people, processes, and ongoing oversight β not just a document.
Starting Point: A Practical Roadmap
- Month 1: Complete an AI inventory. Know what you're running before you govern it.
- Month 2: Assign risk tiers and ownership to each system. The highest-risk systems get structured oversight first.
- Month 3: Draft a governance policy with approval workflows and monitoring requirements. Have legal review for regulatory alignment.
- Ongoing: Quarterly review of high-risk AI systems. Annual policy refresh. Incident review after any AI-related event.
The organizations winning on AI in 2026 aren't moving the fastest. They're moving with the most clarity about what they're doing, who's accountable, and what their exposure is. That clarity starts at the executive level.
